Purpose
This online course is intended to sensitize the user to the importance of using the structured NICHD language to categorize the attributes of FHM tracings and to incorporate this information into the communication between providers and clinical staff during the management of labor and delivery. Awareness of the implications of fetal heart tracings, along with appropriate management can help improve outcomes and promote patient safety, quality of care and reduction in professional liability.
Objectives
At the conclusion of this activity participants should be able to:
Define and describe key fetal heart monitor (FHM) features including the baseline, variability, accelerations and decelerations, and distinguish them from artifacts
Define the basic physiological principles responsible for variations in normal FHM tracings and the diagnostic criteria involved in the recognition of indeterminate and abnormal patterns
Differentiate between respiratory and metabolic acidosis
Critically evaluate the process and methods for the determination of fetal acid base status
Recognize normal, indeterminate and abnormal fetal heart patterns
Explain the indications and contraindications of available therapeutic interventions

Each case study walks users through timed segments of labor management and tests clinical judgment in interpreting and responding to FHM tracings. Clinical information is provided on pop-up narratives to set the stage for each scenario and fetal tracings accompany the clinical information on each screen. As users progress through each timed segment of labor management, the course tests users’ knowledge and provides relevant didactic education. In addition to offering several case studies, the course also includes a Pattern Recognition Workshop that allows users to practice and self-test in recognizing FHM patterns according to the latest NICHD criteria. Throughout the course, references can be accessed at the “View References” tab to expose users to evidence-based literature and resources that support the practice standards included in the course.
“Advisor’s Opinions” tabs enable users to access an audio-video feature where specialists in obstetrics and maternal fetal medicine provide opinions and comments relative to the topics covered in the various sections throughout the course.
